Any school nurses?

I have received a letter this morning from my 11 year olds school nursing team asking me to contact them as they had assessed my child’s Digital wellbeing questionnaire and health review and they would like to talk about some of his responses.
I have no idea what the questionnaire asked or what issues his answers may have raised. Anyone else had similar?

Yes I gave permission last year and he is 11

So was it school specific or
council driven? All children at the school have done? What did they say purpose of it was?

It is a nationwide questionnaire that all year 6 children are encouraged to complete.
I was just curious as to what may trigger concern as it is all about their mental health and well-being.
