Hello, have to fill in a medical form online for a new position in a school.
Towards the end it's asking for evidence of vaccinations, I do t know how I would even go about that, is it correct or am I missing a crucial piece of the puzzle 🤦🏻‍♀️🤦🏻‍♀️

Your GP surgery would have a record of all your immunizations.

You can download a covid jab certificate from the NHS app.

It's things like hepatitis and measles antibodies etc.
i thought it was a quick online form, I can't imagine my GP surgery helping.

Ask your GP. Just a pre warning though as I'v changed GP a few times my vaccination status etc has been lost. If that's the case request a blood test to check for the antibodies and worse case get the vaccines again.

You can ask the receptionists to print them out as they should (!) be documented on the electronic system. You may need to wait a day or so to collect them.

Does tou GP surgery have a patient access portal ( where you can book appointments, order repeat px etc)?

I had to provide evidence of vaccinations for a new nursing position and I was able to download my immunisations on to a PDF file. It went right back to my baby vaccinations in the mid 80s.

Their online facilities are beyond a joke lol.
As it turned out I popped in and asked at reception and she printed them on the spot so that was a worry over nothing! Grin

It's quite a common thing, GP surgeries are the only ones who have your records.
