Burt reading assessment ??????

Has anyone got the score sheet thingy . I have the words and the instructions but no chart to get the results . I have look every where and been on google for ages and I cant find a bloomin thing . Has anyone got it . TIA .

This will be too late for you, but in case anyone does a search on Burts (like me, hence how I came to this thread) and wants the info, the score sheet is after the words on this link.
