Burt reading assessment ?????

Has anyone got the score sheet thingy . I have the words and the instructions but no chart to get the results . I have look every where and been on google for ages and I cant find a

Am I being thick i can find the words . I can read the instructions but I can find the bloomin chart to read the results . Please help . LOL

If your child is under nine years old you start at the top. You get your child to read the words without any help. When they start making lots of errors you stop. You count up the number of words. The table has number of words that you would expect a child of a particular reading age to read.

thats just it .I cant see the table . i have printed out the instructions and the words (LOL) but cannot see the table . I have found the words thingy and it says 3 pages but only 2 show up on my computer and the third page is blank . Please help as I have spent ages looking for this bloomin thing and its driving me nuts .
