D11 or D50k court form for non payment?

Court ordered a sum to be paid to me by EXDH 7 months ago as part of the divorce final settlement.
EXDH has not paid anything nor made any noises that he intends to.
I need to take it back to court, but I'm not sure if it's a D50K form or a D11?
I can't afford to pay anymore mover to a solicitor, I'm up to my eyes in debt already.
Has anyone done the forms to go back to court?

Nope still none the wiser. The court won't discuss anything in person and no response to emails.
