Help! Did I change my name when I got married?

Currently filling out divorce petition and it asks if I changed my name upon marriage.

Options are
No
Yes by marriage certificate
Yes by deed poll
Yes by other

So I definitely became known by married name at work and it's my name for HMRC purposes but on my marriage certificate I can only see my maiden name, no mention of me becoming my husband's surname. I can only find the certified copy of my marriage certificate rather than the original so I don't know if that changes things.

But does anybody know what my name is and how I should fill out the form?

I changed my name at marriage but you wouldn't know from the marriage certificate.

What is your name for things like your bank account, utility bills, etc? The name on your marriage certificate will be your maiden name, but if you’ve used your marriage certificate as proof of marriage when changing your name with other companies, then you would choose “yes by marriage certificate” I assume

Changed by marriage certificate just means (I think) that the marriage certificate mentioning your partner's name is the only evidence (for want of a better word) of how you acquired your married name. My certificate certainly doesn't mention that I adopted my partner's name, even though in some circumstances I have.

It's yes by marriage certificate. This is all the proof you need to change name upon marriage, as it clearly shows your old surname and your husband's surname.
If you had wanted to change it to something else, eg a combination of your names to create a brand new one, you would have needed a deed poll, like any other circumstance for changing a name.

Well your marriage certificate would have your maiden name on.... obviously. Because that's who you were when you married iyswim.

Oh okay thank you, I'm just panicking because I've heard it's really expensive if you make a mistake on your initial divorce petition. I wasn't sure if the certificate was meant to be signed off in my husbands name to signify it being changed. I guess I changed my name by marriage certificate as that's what I did at work / HMRC. Thanks for your help everyone Smile
