Is this for divorce proceedings in the County Court where you intend to replace your solicitor(s) by representing yourself? If so, you can give notice to the Court as per the following and serve a copy on the other side (i.e send it to the respondent if you are the petitioner and vice versa):

Basically, all the court wants to know is where to send documents and presumably your address for service will be same as your home address, but if doesn't have to be if you'd prefer to have correspondence sent to you elsewhere.
NB. Try as I might (and I have many times!) I can't make the form align properly: the case number should be further to the right and Notice of Acting in Person should be centred rather than being on the left hand side of the page.
