CHC funding in Wales

Can I ask for any help or advice re: applying for CHC funding? I'm in Wales and my parent is moving from self funding (for now) residential dementia to self funding (for now) nursing dementia care due to a change in needs. They have a social worker. They have limited savings and own a small house. Wales savings limit for paying for care is £50,000 I think? Higher than England in any case.
But a few people have asked if CHC has been mentioned and it's not so I wanted to find out more before I raise it.

Health issues are:
Mid stage Alzheimer's disease
Type 2 diabetes
Atrial fibrillation
Poor mobility
Multiple falls
Moderate incontinence

Thanks

CHC funding can be notoriously difficult to obtain.
My MIL has just qualified for the funding ( England) and just moved into a nursing home after a month in hospital.

Following some falls at home she now has significant needs and is bed bound.
There was a meeting for family with the Complex Discharge Nurse and her Social Worker.

Nurse told my DH she was “ throwing everything” at the application. It will be reviewed in a few weeks.

The nursing home should be able to advise you if they have CHC triggers, which will entitle you to an assessment. Are you getting the FNC element paid at the moment? Be sure to check you are.

She would be entitled to FNC if needs a nursing home. Has a nursing needs assessment been completed?
