Do ties need washing?

I reckon we have some 20 year-old jobbies that have never been washed.

Is this normal or disgusting?

Ties go out of fashion so quick I am suprised that anybody is wearing one from 20 years ago, I still have them but they are on the tie rack of shame hidden at the back of the wardrobe.

All male Obs & Gynae consults, regs, & SHOs have open-necked shirts. Often one button open too many IMO. With chest hair poking out.

You should go to Cologne for Karneval and wear one of your old ties. Has to be on the Thursday of Karneval, Weiber Fastnacht where the women are in charge of Germany. They will delight in cutting your tie off.
