Spanish Health Travel from please help!

Has anyone filled out the SPTH Spanish health passenger travel form before flying to Spain please?!
I am not sure what to put in each section....
Region/Autonomos City:
Province:
City/Town:

The destination is Tenerife, Costa Adeje 🤯

That info should all be on the postal address of your accommodation - regions are like countries in the uk (a little smaller) provinces like counties and city is a word used for any size town or city. Region is probably: canarias

If you click the drop downs on the order they are it should give you options to choose from, probably: cañarías / Islas Canarias, Tenerife, costa Adeje

It’ll be canarias, and Santa Cruz, but not sure which is for which box. Check the drop downs. Not sure about the third one, off I remember rightly only two of the fields are mandatory so
I left the third one blank.
