Covid vaccine certificate different to passport name

I’m wondering if anyone else has encountered this problem. We are due to travel to Portugal next Thursday ( 29th) - a holiday we rolled over from last year.
I have had both my vaccinations and have downloaded my Covid vaccine certificate on the NHS App. For Portugal , proof of having had both vaccines is required to avoid quarantine on entry.
My problem is that the Covid vaccine certificate is in my married name ( as my NHS /GP records are in that name). However, professionally at work I retained my maiden name when I married and I kept my passport in my maiden name as I used to travel a lot with work pre Covid. It’s too late to get a new passport. My GP says they can’t change my name on their records back to my maiden name ( which seemed to me to be the easiest solution so both names match up and is what the government website suggests needs to happen ).
Nobody seems to be able to give me a straight answer on how to resolve this but I can’t be the only one that is in this situation! Just wondering if anyone on Mumsnet has had this issue and how they got around it? I plan to bring my marriage certificate with me but it is stressing me out!

Get your GP to change the surname on your medical records to the surname that matches your ID (obvs you can produce the ID to verify this).
Then generate another vaccine passport.

I had a similar issue with travel to Portugal a few weeks ago, although in my case I had the shortened version of my name on my PCR results, not my passport name.
I was in a tizz about it before hand so copied and pasted standard statutory declaration wording and got my boss to sign it saying that she had seen a copy of my passport but XX was the name I go by at work, and what is held on my HR file.
At Portuguese immigration the lady did look at the document and said she wondered why the name was different, but waved me through, It wasn't clear to me whether she would have been satisfied with my verbal explanation or whether the note from my boss had swung it as it didn't technically meet the requirements for a stat dec - but I was very glad that I had taken it!

Does your passport have your other name recorded too? Mine does, you can have also known as xxxxxxx put in as an official note.

Unfortunately it's too late to change my passport to add maiden name and I dont want to change my medical records if poss as I'd have to change them back!
Just hoping someone may have traveled to France using marriage certificate as proof of 2 names?

I’m travelling to Spain on Tuesday and have just realised my passport is in my maiden name and vaccine passport is in my married name (I’ve changed my name at the doctors but haven’t got round to changing my passport). I don’t think changing my name at the doctors is gong to be an option due to the timescales involved . As it’s Ryanair it’s impossible to get through to ask the question of the airline. So - has anyone in this situation successfully traveled showing other supporting documentation to prove iD - marriage certificate/driving license etc?

Hi I have the same problem. Travelling to Paris next week. My passport is in my maiden name and GP saying I either need proof of divorce or change by deed poll! So frustrating. Did you get through with the marriage certificate? Thank you

I'm in the same boat, covid cert is my married name but passport still in my maiden name. I flew with ryanair recently and brought a copy of my marriage cert just in case. Covid cert was only glanced at and no one mentioned the different surnames. Hope everyone else with the same dilemma manages okay!

Can't use verifly because my passports aren't same name as NHS knows me by. I'll let you know what airport says by Friday!!

I had this exact problem a few weeks ago.
Definitely persevere with your GP.
In my case, I was getting the Eurostar within 48 hours of going to my GP to ask them to change it, and was stressing about it, as the receptionist said she would change it on their system, but that it would take several weeks to link up with my NHS records on the app/the Covid records.
I looked on the app the day after, and it had miraculously changed! 👍

I changed mine at the drs and it appeared on my passport later that day. I was very impressed at how quick it was changed!

My GP/NHS records accidently got changed to my married name. I just emailed the practice and they changed it back to my legal name- wasn't an issue.

I'm just amazed at anyone who got their GP to pick up the phone much less do something.

@lljkk I went in to the GP surgery instead of calling, as knew it would take ages to get through on the phone.

This is such a sexist problem. What a ridiculous thing for the vaccine passport people not to have thought of!
I was thinking it was the GPs who are being sexist too.

ps: positive response from GP surgery today, walked in. I filled in a form to name change NHS. X fingers will be sorted quickly.

pps: my NHS login now shows my maiden name (to match my passport) in medical records section (GP records) but not for the covid jab/travel certificate. So that's nicely inconsistent. Oh well...
