Yes, but I'm 35.
I give them and don't usually bother asking women over 60 even though we're meant to ask everyone (I don't ask men either!).

In the screening/triage stage before the actual jab I (and everyone else I presume!) had to give an answer to a list of printed questions stuck to the Perspex screen including ‘are you or could you be pregnant?’

It is on the standard list of screening questions for everyone getting the vaccine. Some assessors will just assume, I tend to ask everyone, or will make a comment to say "you're not pregnant" as I tick the no box if I'm absolutely certain it's not a possibility (e.g an elderly gentleman).

I was very obviously pregnant when I had my vaccine.
Consenting Drs talked very thoroughly about my decision and understanding of risk v benefits before signing the forms.

I vaccinate and I ask this but I only ask women of childbearing age- under 55.
Not had that many of those so far.

We were given a form to complete. One question asked if you were pregnant, breast feeding or planning to become pregnant in the next 3 months.

I vaccinate people. They have to tick no on a consent form before we'll vaccinate them.

It was on the consent forms I filled in - which are standard for everyone, so obviously some questions are obvious "no" answers for one sex or certain age groups. The consent forms were quite extensive, but I'm not in the UK.

We weren't asked anything verbally except whether we felt well and whether we had had any covid symptoms within the last week.
