High risk, off work, evidence for employer?

I wonder if anyone can help.

As expected, I received a high risk text on Monday due to severe asthma.

My line manager sent me home for 12 weeks straight away. I've since spoken to HR who have said I can self certify for the first 7 days, then require a medical certificate for the other 11 weeks.

I've phoned my GP surgery who have said this needs to be done through the .gov website. All I can find on there is how to get a 7-14 day isolation note if I'm off work because I or someone in my house has symptoms.

Am I just missing what I'm supposed to be completing online, or has the receptionist misunderstood what I need?
