Both! 3 years of trying with no success, NHS tests came back as unexplained infertility so we were referred for IVF (the clinic immediately confirmed I actually have PCOS). We conceived from our first round with one healthy embryo and none left to freeze for further transfers.
Fast forward 2 years and I am now 15 weeks pregnant again naturally, to our complete and utter shock as we had decided not to go through the heartache of trying again!

2 accidents, miscarriages.

conceived first time trying and have a 7month old.

pcos diagnosed via scan and bloods.

No trouble conceiving naturally, three babies. Taking inositol and then getting down to (and maintaining) BMI 23, made my periods regular and pcos symptoms disappeared. I also didn't drink alcohol whilst TTC because this can affect hormones more severely in pcos women than in normal women.

The healthy weight is crucial, plus inositol, breaks the vicious circle (insulin/testosterone etc). There will of course be plenty of exceptions for whom it doesn't work, and conversely there will be obese pcos women who conceived easily. But to maximise your chances - maintain healthy weight, take inositol, don't drink.

I am one of those who inositol and weight management did not work for. My weight was a very healthy 21-23 during our 3 years trying, which included taking inositol and I never drink anyway.
Now I'm off inositol, considerably heavier after DD1 (I think BMI of maybe 29-30) and conceived naturally.

There are definitely success stories of managing PCOS symptoms with diet and supplements, but as PP said, it can still happen without

Edited to add; cutting out dairy had a MASSIVE improvement on my PCOS too. There’s lots of research about dairy and PCOS.

Naturally. Conceived first time for all three DC. I got gestational diabetes for all pregnancies which I was told was related to PCOS.

Naturally and very easily to my surprise, as in my teenage years I would go months without periods and then I was on the pill for years for contraception and acne treatment, the doctor warned me I may have problems getting pregnant, anyway came off the pill and conceived straight away each time for 3 pregnancies.

Naturally. Prior to conceiving my first, I highly expected I would need medical intervention in order to conceive, as I would go 8-9 months without a period. However, we got lucky, and both of my babies were conceived naturally (my cycle had become more regular after my first child).

Naturally, on a vegan diet. Second the poster above who said cutting out dairy led to a massive improvement in PCOS symptoms.

1 dc through ivf after numerous tests and being told I had no other choice but ivf - no "let's try x, y or z first, ivf or nothing).

1 dc naturally 9 yrs later - no idea how that was possible!

This was 26 years ago mind - terrible periods delayed for months, horrendous PMS, chin hairs and had still had acne at 30 but I was a reasonably healthy BMI (24) and conceived naturally and quickly.
