IVF with low AFC AMH - advise and recommendations please

I’m 38 with low AFC, low AMH and high FSH. TTC for 4 years and have history of 1st trimester miscarriage in 2020.
In April my results were AFC:3, AMH: 2.9, FSH: 9.7 and which has worsened further in Oct – AFC: 2, AMH: 0.7 and FSH: 14.9. In July we tried first round of IVF at Guy’s hospital and had to cancel the cycle mid-way as I was not responding well and had only one follicle. Since then, my cycle has been messed up and we’ve not been able to have another go at IVF on NHS.

We’ve not had a great experience at Guy’s hospital and considering if we should go private for another go at IVF before we consider using an egg donor. The whole process has been dragged for too long and having a toll on my mental health. We have funds saved for couple of rounds of IVF privately, but not feeling positive if there is any hope given my diminishing reports.

Looking for advise on if its worth giving another shot at IVF privately using my own or if we should go with an egg donor. Also, any recommendations for clinics in London that have good results with low ovarian reserve will be greatly appreciated. I’ve seen a few posts that have suggested Create for low ovarian reserve, and wondering if it will be worth it to have an initial consultation with them. Any idea how much private clinics charge for an initial consultations in London?

Hi there, I just thought I'd reply as I'm in a similar position - but not sure how much help I'll be!

I'm 37 with AFC between 1-5, AMH 0.5 and FSH 9. I've just finished my first cycle of IVF on the NHS at Chelsea & Westminster in their Assisted Conception Unit and I had a great experience there and felt very well looked after. They only got two eggs, and one fertilised and made it to day 3, but sadly it didn't make it to day 5. So we are back to square one and looking at private packages too - but given how well treated I felt at C&W, I made a private appointment to see one of the Consultants - Julian Norman-Taylor - and he was so so helpful. It was £230 for 30 mins, and we are definitely going to do the three-cycle package with him (they also offer a refund if you get pregnant on the first cycle). He said I do have eggs, it's just a question of getting them out at the right time to make sure whatever does come out can fertilise - which actually sounds kind of similar to what Create are saying anyway. For women with low AMH, it's about focusing on quality rather than quantity.

I did look at Create and will almost certainly give them a try if I don't get pregnant in the three-cycle package with C&W (if I can face more IVF)!

Good luck with it all - it's unbelievably stressful, so look after yourself as much as possible!!
