TTC, low AMH and endometriosis

Hi all,

This is my first post on here so getting the hang of it 😀

I have been TTC since July 2022. After no success after 6 months using ovulation tests (showing I was ovulating each month) I went with my gut and I got some fertility bloods done in Jan 23 and was told all was ok. Still no success so in May 23, I went to a consultant to see if I could try anything else, he suggested a laparoscopy to see if tubes blocked, thyroid or endo. Got this done and he actually found endometriosis in my pouch of Douglas and he did endometriosis excision there and then. I never had any symptoms of endometriosis so this was a shock. Following cycle I got pregnant but it was short lived and I had a chemical pregnancy at 5 weeks. 3 cycles later I went to a fertility consultant who suggested I have a Folicle tracking scan, I got one today CD9 (he couldn’t do it CD10-12) and it measured 12mm. He said there was nothing he could do this cycle as it should get to 18/20mm but it won’t. He basically said I have zero chance this cycle. Is it not too early for him to have written this cycle off? As I will still ovulate?

I had my AMH done in Jan 23 and it was 10.20 nothing flagged as an issue and I didn’t realise it was low. Then todays results showed it was now 5.7 (apparently surgery for endo removal can make it lower). So as a 32 year old, I now have classed ‘low’ egg reserve, roughly same level as someone who is a lot older. I was also told today that IVF is only a 33% success rate for risking €7k - words of the consultant not mine. Naturally I am very upset now and don’t know what to do next? Anyone have similar stories or hope?
Finally, he has prescribed me (for my next cycle, well x4 months) Letrozole tablets and Gonasi. Any advise or stories would be really appreciated!

I don't know much about endometriosis or AMH. But I have had 8 rounds of IUI and a 12mm follicle on day 9 really doesn't seem that abnormal to me!

Mine is usually measuring very slightly larger than that by day 9 but really not much - sometimes 13.5 or so - and I've never been told there are any concerns. I log all my scans and there was one time I was 12mm on day 9 and they didn't say anything - the cycle went ahead and it was big enough by the time I ovulated.

Have you tried taking OPK tests at home to check you are getting your LH surge and ovulating? And are your cycles a regular length?

I am no Dr but I really didn’t see how 12mm on CD9 was an issue if they grow 1-2mm per day so really confused about why he’s just said this month is a write off already!

I have used the clear blue 4 day in advance ov tests where I always got my blank, flashing and static smiley. After spending a small fortune on these I now use the easy@home ov sticks and always catch my LH surge and I monitor BBT too where I catch it, so I am definitely ovulating and bloods have confirmed this too.

I also am regularly doing acupuncture and my progesterone was 62.50 in recent bloods which I believe is good! Will do anything else to help and trying to remain positive that yes I have low egg count, but it only takes one egg 🙏
