Low LH/FSH ratio, High Prolactin TTC Aged 38

Im currently on month 9 of TTC (aged 38) with no hint of a positive test and have just had my initial blood test results back:

FSH 7.6
LH 5.3
Progesterone (day 21) 42
Prolactin 776

Due to my age the one I was most worried about was FSH - that seems to be ok (good even) but my prolactin levels are a fair bit above what they should be and I'm a little concerned about my LH/FSH ratio which I believe should be between 1 and 2 and mine is around 0.7.

Just wondering if anyone else has had similar results and what the outcome was, were you able to conceive naturally? I've got no idea what's causing the high prolactin although I have been told by the doctor that stress can raise it so I'm having a retest in a month. I'm a bit worried (after googling) that the high prolactin could be making my fsh levels artificially low and also after googling worried that the lh/fsh ratio could indicate low ovarian reserve 😬

For reference, my cycles are regular and vary between 24-26 days, usually 25 days (this short cycle length also concerns me as over the last few years it has gone a couple of days shorter!) and I usually spot a small amount a few days before my period is due. My O/H has had a semen analysis done and will get his results tomorrow.

Can anyone relate/advise? 🙂

I just thought I’d reply because I’m in exactly the same boat as you. I’ve just turned 38 and have been ttc for 12 months and have high prolactin.

I’ve had my day 2 and day 21 blood tests and they are as follows:

FSH 7.1
LH 8.4
Prolactin 1095
Progesterone (day 21) 48.8

Apparently my FSH/LH ratio is 2.1 but the fertility consultant seemed to think that was ok. I’ve had my prolactin retested and the second time it was 1403 so I’m now seeing an endocrinologist next week.

I also spot for five days before my period. Had scans done and gynaecologist says all is ok so I’m wondering if the prolactin is causing this because it does seem odd to me.

Hoping someone will come along and say they’ve conceived naturally with high prolactin! Although from my reading online I’m hoping it’s easy to fix.

Any update from you guys

Dreaminginpink · 05/11/2023 00:58

@DevonDancer @tife1234 just as an update, I had my prolactin retested and it had come down to well within normal limits so they said it was likely down to stress or something 🤷‍♀️ So that was kind of good news but I'm now on cycle 11 and still no hint of a positive test. I've been referred to a fertility clinic but been told the wait is almost a year, I'll be 39 and a half by then so need to have a think about what to do next! How did you get on @DevonDancer, have you seen the endocrinologist yet?

@Dreaminginpink thanks for updating me

DevonDancer · 07/11/2023 21:23

@Dreaminginpink @tife1234 I've had my appointment and they have referred me for an MRI scan.

I've ended up doing my fertility tests privately to speed things up!

Just thought I would comment on my old thread to give anyone else in the same position as I was in some hope! I finally conceived in month 15 and now have a healthy baby boy who is 6 months old tomorrow 😊 I didn't do anything different on the month I conceived - the only thing that had happened was I had been in for a procedure to remove a polyp from my womb - the procedure was abandoned as it caused too much pain when they were trying to get through my cervix. Maybe that opened something up, who knows! The polyp was never removed as I got pregnant that month! So, don't give up hope, even when you reach the scary 12 month mark. That was my first positive test as well!

Thanks @Dreaminginpink I’ve just had my bloods back today, I am still waiting for AMH & Oestrogen but my FSH was 6.5 & LH was like 4.4. So I’ve been told all normal but my LH to FSH ratio is 0.67 and now I am worried. I am 5 cycles down, no hint of a BFP. Semen came back good and so did my progesterone. We had a chemical pregnancy last August. I ovulate regularly every month and I am literally not sure why it’s not happened.

I know 5 cycles is nothing but still I can’t help but analyse every little result. I didn’t know LH to FSH ratio was even a thing!!
