I got a Laithwaites £40 voucher code and it worked then removed it and says it's already applied and won't add it.

Some others had this issue too and customer service asked for the email etc of the person who used my code (which I don't have).

I had similar recently with a bonus I received from Papier after another MNer used my referral code (weirdly I was able to use my one from Laithwaites) and posterd here for advice ...

Another lovely poster (apologies I can't remember who) said they all use the "Mention Me" thing (Hush use it too) which isn't very helpful but does encourage "sharing " of codes so we aren't breaking rules per say.

I got round it with Papier - so hopefully Laithwaites will be similar - by saying the person who used my code is someone at work I only know vaguely so wouldn't be comfortable asking for her details. I did have an email from them saying "congratulations your friend has used your code". They asked me to set an actual account with them (I'd just used guest checkout when using the code I'd been given for my first order), provided them with their email and they were able to credit.

I hope that makes sense? Most of my rambling doesn't

As explained above ... really important that you click through the retailer you want to shop from and be aware that using a promo code too cash back haven't approved means it may not track. Always think of it as a bonus if it does. They will often sow codes they approve so that's helpful.
