Any ideas?

My Aunt is mid 80’s with dementia. Doesnt really leave the house. In the past I have done chocolates/treats, gloves, the usual smellies.

what am I missing?

Grateful thanks for any ideas/advice.

Can you make her a book and put all old photographs in ? I have done this and it was a huge hit. We always look through it on visits and on the old occasion she will say a memory from them.

The care home residents I worked in loved cuddling a doll.

Digital photo frame. Tom's guide has a great list of them, and you can make a slideshow of family pictures that would play on it.
Or a custom photo book, see Shutterfly for that, they have really nice templates and designs. Add some brief info about who are there in those photos, and you can even colorize old black and white photos to make them brighter and more recognizable for her.

I can particularly recommend picture books. We got late DMIL one from here called "It's a Funny Old World" and she spent ages with it. She especially liked the pictures of babies and small children and responded very well to them.
