It could be reflux. My 14mo DD has reflux. When its bad she coughs at night and there is a different smell.

I don't think lying on back will help. It might make the acid sit in the back of the throat and cause more discomfort.

My 14mo dd has suspected silent reflux, but didn't seem to have it when a young baby - is that possible? She's on losec, but threw it up this morning, and it's hard to get it down her. She writhes around and arches her back in agony but only at night. Tried so many doctors and remedies, but it seems to be getting worse.

Yes, she has bad breath.

However, cough could be another illness/virus which causes bad breath. If so it will pass in few days / a week. If doesn't go away, then could be reflux. If it's reflux sleeping on a slight slope (pillow under cot mattress at head end) helps.
