Baby dropping percentiles going round in circles

Feel like not getting anywhere, my ds is 12 weeks old today , he was born on the 75th percentile and is now below the 0.4th he is mainly breastfed every two hours with two bottles of 4oz express bread milk a day (which he never full finishes)
over last 12 week struggled with him gaining weight. He gains slowly, remains static or losses weigh and seems to go in this pattern. Over this time he had a water infection and tongue tide he also suffer with wind and reflux offer bring up milk , I have tried top up feeds with formula as suggest by dr , when tongue tie was sorted my supply increase and swapped them out for Breastmilk and he seem to feed better but his weight has still followed same pattern
he was under consultant but they are off sick he a has weigh in twice a week and they put him on Omeprazole which doesn't seem to work , I have ask for a high calorie formula/ or Breastmilk fortifier to give once or twice a day however we keep getting fob ff with increase in Omeprazole and try normal formula or we’ll see at next weight check , feel like going around in circles

If there's a sense your consultant's sick leave will be for a long time, with a small baby it seems reasonable to contact their secretary expressing your concern and asking if someone else could see your child in the meantime.

This is very worrying and you must be beside yourself.

If the consultant and good precise hospital-based medical care aren't available then I would go to A&E. The 0.4th percentile is very very small and not somewhere that a child born at a healthy weight should be at at 12 weeks.

Before you even get to A&E I would offer a near-constant supply of formula in a bottle as well as breastfeeding. It's easy to be emotionally attached to breastfeeding but in this situation the only important thing is to make sure your baby is getting as many calories as possible while you wait for an explanation. There may or may not (hopefully be an underlying allergy or metabolic disease but not gaining weight at this age very harmful and could have long-term physical and neurological consequences.

DS2 was like this and the health visitor later apologised to me for not noticing he had 'failed to thrive' and hadn't taken my concerns seriously. She weighed him, watched him feed and re-weighed him and yet he'd barely put on any grams for the ounces of milk he'd had. I kept telling her that he'd soak a muslin bib at each feed and his neck folds were sore and smelly from all the milk that kept coming back out. It was a reflux problem and we were referred to hospital consultant shortly after.

Th man you for your replys Spoken to consultant secretary to outline our concern's especially after speaking to a dietician (which he should have been referred to well before now ) consultant wont be back till December
also asked for a move to different consultant and a review of his case as it seem he’s been forgotten about apparently there is quit a wait to move consultants as there is a lot off sick we’ve had home at a&e twice , we’ve raised concerns and and try to be proactive in dealing with this but always meet with blank faces and repeating our selves and his case
I’m feeding him constantly including bottles of expressed breast milk stop formulation top up and replace with bottles of express Breastmilk in case of CMPA ( cut out dairy in my diet completely years ago for lactose intolerance)

Does your area have an infant feeding team? This sounds really stressful and worrying. If you do have an infant feeding team I’d be in contact with them ASAP
