VERY cloudy urine but clear on dip test - what could it be?

Dr Google isn't helping me here, but maybe one of you can.

DS (11) is ill, but the dr can't work out what is going on.

He had to do a urine sample, which tested clear for 'everything' when dipped, but which was incredibly cloudy. I've never seen a sample like it. After about 10 mins it separated (suspended solids below, clear above).

Any ideas what might cause this? Bloods are clear too.

Dipstick tests are not entirely reliable. If his urine is cloudy and he has symptoms I'd ask for the test to be sent to a lab to see if it grows anything. Even then, infections can be missed.
