Heat rash or scarlet fever?

About 3 weeks ago DS (4) had a rough red rash at the top of his back. I assumed it was heat rash or chafing from his t-shirt but the next day it was all over his back and he had a temperature although he didn't seem unwell otherwise. Called 111 and had a video call with a dr who diagnosed scarlet fever and prescribed amoxicillin. We did miss one dose towards the end of the course, which I felt terrible about.

This evening the rash is back, the same rash in exactly the same place it started last time. He doesn't have a temperature and is totally fine in himself.

Is it possible that this could be scarlet fever coming back again so quickly, and if so is that my fault for missing a dose of antibiotics? Or is it more likely that both incidents were actually heat rash and the temperature he had last time was just a coincidence? His symptoms were very mild...

Highly unlikely missing one dose
at the end would make a diffeence, more likely to be something else especially if no temperature and seems well.

What does the rask look like OP, my ds had one a few weeks ago that kept coming and going. The ooh doc he saw said it was viral.
