17 month not pooped for over a week?

Just looking for some advice really, until I can talk to GP tomorrow (hopefully).
My son is 16 month and just checked with partner and neither of us have changed a dirty nappy for well over a week.

He's been straining for the past three hours but all that's there is a smear. He previously had some lactulose prescribed at around 6 months as he had such hard poop he had a fissure but hasn't really had any issues since then.
I have some paediatric Movicol from my daughter who has chronic constipation but dont know if he's allowed that?
