Struggling with reluctant pooer with tearing.

Our 3 year old DS doesn't like doing poohs. This seems to have come on since he had a bout of constipation some months ago. He has always been a nervous pooer anyway and never poohs unless we are in our own home

I have a feeling that he is holding on to his poohs and only doing them when he is completely compelled to. I'm sure this just makes it worse.

We've given him syrup of figs to help him out with this and to make them softer.

This morning he had a really sore bum with some blood. This has never happened before so I put some sudocrem on it. I assumed it was nappy rash. He's never had nappy rash either so I didn't really know what I was looking for.

He went to nursery today and we forewarned the nursery that he would need some help with the nappy rash.

Today when my husband went to pick him up, the nursery assistant said she didn't think it was nappy rash but that it was tearing instead.

He is a scratcher too and I have noticed him scratching his bum once or twice before.

I have no idea what to do next. Do I take him to the GPs? How do I encourage him to not hold on to his poohs?
