25mth old with v sore bottom & cant poo - what is it?

my poor little DS has been constipated & distressed for a few weeks, which is very unusual for him, as he's usually regular as clockwork, same time, every day.

Its making him irritable, upset, and he cant sleep as his tummy hurts. I've been giving him Lactulose every day, and today Fig syrup which has had no effect.

I thought it was constipation, but having had a quick look at his bottom now he finally went to sleep, the area round his anus looks red, sore, with little white bits embedded in the wrinkles (sorry but hoping detailed description rings a bell with someone), which didnt come off when i gently rubbed, and the poor love flinched in his sleep. Also looked kind of moist down there, which i think it shouldn't be? It would help if i had any frame of reference but i've never looked closely there as havent needed to.

anyway, after seeing that i've started to think maybe the constipation is a symptom not the cause? could he have an infection there? or thrush? or something else?

anyone got any ideas? i will phone the GP tomorrow but would like advice as i am disabled and it will be extremely painful for me to get to the gp with him, but don't want to send him with someone else if its something serious/ not easy to diagnose, but if its something the doc will have seen a thousand times before, then will send him with my pa,
