Grainy or sandy poo in toddler (17 months old)

My ds has had this on and off for many months and I was wondering if anyone else had experienced this and could help shed some light on what it may be.
The sandy or grittiness is really hard to wipe off his bum, often causes rash (so maybe acidic). He's fine in himself, eats well, but has these strange nappies quite often. Any ideas or is a trip to the docs?
