'Child health surveillance' tick box on GP registration form

Just registering with GP in new area... on the registration form there is a tick box for registering a child under 5 that says 'I wish the child above to be registered with the doctor named overleaf for Child Health Surveillance'.

No idea what this means, can anyone help? Sounds scary!

Yes, its registering with the HV

It's agreeing that you would like to be registered for the Health Visiting service, and immunisations.
