CM help on parent questionairre???

I am putting together a questionarre for parents of my mindees to do, so i can show OFSTED at my next inspection.

Can you give me a little advice on what to ask and tell me what you think so far?

here it is...

Dear Parent/carer,

I would like to offer you the opportunity to share your views with OFSTED. My next inspection is due this summer and I would like to present the inspector with parents opinions, in order to give them a greater insight into the day-to-day care I provide.

Would you please complete the following questionnaire and return it to me.

1. Did you find the welcome pack helpful? Was there any information you would have liked to see included that I have missed?

1. Do you like the play rooms and garden? Is there anything about the environment that you thought was particularly good, or could be improved?

1. Were you pleased with our communication and feedback? (i.e. home books)

1. Do you feel that I provided a wide range of activities and outings?

1. Does your child give feedback at home? Is it positive or negative?

1. Do you feel that your parental wishes were always met?

1. Would you recommend me to other parents?

1. Do you think my prices are reasonable?

How about using a few more EYFS words (I see you have already used environment). How about asking what they like most and/or what they would most like to see continued/changed?

could on of you really good ladies just word one for me so I could print it off to use.. lots of E.Y.F.S. stuff and make it sound the biz.. thanks in anticipation..!!
