CM club, where do you get your 'injury books' from?

Hi, i need to get some new 'bump' books for our pre-school. The ones where you note down that "Jimmy hurt his knee and we washed it with water" and then the parent signs it and has a copy.
Is it just NCMA that has them, or can you get them anywhere?

As a cm, I have always used all ncma stationery but you can probably get 'accident and injury' books from good stationery wholesalers.
