Nanny payslip advice please

We have a Fab nanny who has been with us for the last 2 months.Although we have paid her and her tax etc., for this period we have not given her any payslip.She needs it urgently as she was without job for the last 5months and she can get some financial help from Shaws trust. Does any one have a payslip template which is user friendly.I need it ASAP as the deadline is in 2 days.TIA

Something like this - may not need to be that complex though. List of what it must contain can be found at Direct.gov.uk

A simple payslip could be as follows:

Employees Name
Date (for example 1 April 2009)

Gross Pay:
Employees NICs:
Tax Deducted:

Total Pay (Net):

May be handy to add Tax Code, Employees NI number.

i just do one which has her name, tax office reference, date, gross, NI and tax deducted and total net pay
it also has total pay to date this year and total tax to date this year (ie total added up of all payslips this financial year, iyswim).
if the total varies from usual, usually add a brief line to explain why (eg overtime for...)
