Application Forms for CMs - who uses them and what do you put on them?

On the back of another thread just wondered who uses application forms for parents wanting to use their services. I just heard someone else using them and though great idea - but what do you put on them?

I introduced them to motivate parents to specify which days/hours they need and to make even clearer that I'm not saving any days until I have their deposit and form. In the past I've come across parents who have been very uncertain about the days/hours they needed but still wanted the certainty of a place being available when they decide which days they need! I can't work like that.

I won't bore you with all the problems I've had before, suffice to say I've been misled by parents and made costly mistakes, the application form has been the solution to those problems for me.

gosh that is so great - i wish I'd had this 2 years ago - another costly mistake!!

I am going to plagerise happily from now on for all new enquiries!
