In my world:-

Long term planning is one page with the months down one side and a column for the theme we are going to cover for each month, any festivals that fall into each month - very basic

Medium planning - I do each quarter and breaks down our monthly theme into the 6 areas of EYFS.

Short term - weekly planning - activities that we plan to cover for that week both in the current theme and individual to each child, showing and that fit to the EYFS areas
