a questionaire type form for a parent of mine who's ds is leaving, but i need something that 'asks' a closed question ie: are you ahppy with the care x has provided...name two good things your dd/ds has learned since being with xx..that kind of thing?

but off top of my head:
how long was your child with x?
was x welcoming?
did the setting feel welcoming?
where you able to discuss your childs progress with x?
did you feel your child had a varied/ healthly diet at x?
were you happy with your childs progress?
did you feel your child was in a loving happy enviroment?
where you able to discuss concerns with x?
did x take your concerns with action?
in your opoinion did you child have enough outdoor time/did your child go on outings/toddler groups?

thankyou DMO..its the parent who's pulling their ds out as she said i 'wasnt flexible' anymore(utter rubbish and she knows it now, she DIDnt come to me to explain what SHE thought was happening and just pulled him out-even threatening nott to pay and for me to take her to court!) so would rather keep it about things other than speaking to me!..although it really was her prob that she didnt as i ALWYAS say on newsletters and in contract 'ANY probs' please speak to me asap?...

dont send her a queistonaire not everybody needs to fill one in just the ones you like

Welcome chick, are you ok today?...i wanted to get a reference type quiestionaire from parent as ive got one from other two who left and like to be able to show we left it on good terms, but obv i want to influence what they say

i wouldn't bother with questionaire/reference - you don't need to have one from every parent that leaves surely?
i'd just let it go - she sounds nasty

immature and sly are words id use, obv thought i was greener than i was cabbage looking!..but kinda want her to 'admit' her ds LOVED it here, like a 'you silly silly girl' type of questionaire..but will alos benefit me if she says nice things
