Ex left DD 5yo alone locked in house

It's been an immaturely regular conversation for years and he has been warned that it is not safe to leave a child alone at home while he nips to the shop.
But then DD 5 told me there was a secret between them that she wanted to share.
After approaching him calmly to address this major issue he basically shrugged it off. It worked out fine for him. He says he was just 10 minutes, I will never know.
Previous red flag: Regarding a play date, he wanted to drop her off with a classmate whom we are not acquainted with. That too was a solid no from me, he needed to attend also and did at a push.
Where do I go from here?Do I now go straight to court & request supervised visitation or is there some middle ground option?
Other than this, there is a wonderful relationship between them. I wish I could I still a sense of child protection and safety. But I am prepared to go visit a solicitor this week.
I feel that my ex can not be trusted. Is there a way through this? I am so deep in a haze of anger, concern and upset.

YANBU unreasonable about the leaving alone thing, I’m not sure I’m with you on the play date but I suppose I would have a sense of the child/family/parent from school drop off so it probably wouldn’t feel like a complete unknown.
im sorry I can’t help with the legal side but make a note of everything, keep copies of messages/emails about it etc.
