CM CLUB: Mindee left over a year ago, coming back for ADHOC care - PERMISSION FORMS.........

Hello

Mindee came as a baby and when parents needed to increase days and I couldn't do, they left and went to nursery. From time to time I've had phone calls to see if I can help out but I've been full. Anyway, with my changes of mindees, I'm now able to help and I've been asked to do 3 months care (just 1 day a week whilst mum on maternity leave) between now and me taking maternity leave. I have all the permission forms from when she was a baby (left about 20 months ago), I presume these are still valid? Don't want to make her fill out lots more paperwork again but thought I'd better check first.

I re-new my permission slips every 12 months anyway - so that doesn't bode well does it

Do the original ones have the dates on them? Why not just ask her to resign and date the original ones. (If no date on them originally just put the old date in brackets next to the first signature.)

Check you haven't implemented any other permision slips since then too!

All I do is ask the parent to read through the old ones and amend anything thats changed, in particular check emergency contact details - mobile & work phone numbers/extensions etc and make sure you ask all about their health etc. If the child's developed allergies or asthma or something, parents don't always remember that this has happened after you last provided care. If there's room to amend the original form then I'm happy with that. If they need to make a lot of changes or I have updated/changed the form only then do I give them a new form.

Thanks. I've asked about allergies, contact details etc etc, it was more the photo permission forms, outings and the usual lot. I think I'll just get a them re-signed
