Child Injury Form (the one with sketch of child)

I am trying to find the Child Injury Form which is often used at nurseries. It has outlines of children on it, so that injury location can be marked.

Anyone got one? Anyone able to provide a link so I can download one?

Couldn't find anything suitable... but did locate some more general bodymaps.

Have created my own, rather simplistic form.
Do you think it will do, for recording pre-existing and new injuries?

Good. I need it due to supervising a child contact session. Not done one of those before... may not be that nice I get the feeling, having talked with one of the solicitors involved this morning.
