three tiny bloody section they need to fill in but £74 is what they charge!

thing is if had been able to see my reg gp, am sure she would have done it free/cheaper too..but she off sick!..so receptionist obviously went by the book

I had to pay £60 and it was the receptionist that bloody completed it from records and got another dr to sign it!

..thats terrible!..they state it has to be gp!..bet she had a good night out!

are you part of local CIS or surestart?..i am and they are refunding it for me

you can go to a myraid of professionals to do this kind of thing can't you?
eg,
teachers,
engineers
ministers of the church, rabbis, etc.

if you have children, their teacher could sign for you.

so can civil servants

no not a medical form, it has to be doctor

i took the form in wednesday morning..i just rang to ask if it had been sent off to ofsted...receptionist told me it had been sent to a doctor in their other branch!!???..and that it would take up to a week before it would be done!!!for gods sake!..i said for £74 youd think shed had filled it in straight away as it was only two questions, she sid 'well they do have other paperwork to fill in too you know'!!!!..yeah but they didnt pay her £74 quid!!!

just rang gp to see if form sent off..NO!..am awiting for secretary to ring me back to tell me whats on it before its copied and posted!!!!!..OMG i hope it says im ok and doesnt dwell on ad's that im no longer taking

doc has said she hasnt prescribed ad's since early this year as they did no good and i am working through myy probs myself and via counselling, she put that she think s i would have no problem and would be good to work with children!

Anti depressants have no bearing on being a childminder evesmama. i am on them and have been childminding for 2 years! FWIW i think you have been grossly over charged for the signatures, i paid £32 for mine.
