Hi, I'm doing a new contract for a baby starting next week and I'd appreciate advice on filling it in!

Also what other forms do I need to get the parents to complete?

my minds a blank.......

I'm in Scotland BTW!

Hi ssd, I always get parents to fill out a photo permission form and a parental permission form (from the back of the ncma handbooks) this covers outings,tranporting in a vehicle and application of suncream.

Late payment charge
Late collection charge
Standing order to go into your bank
Trail period
Form for giving Calpol???(not well thought of - but essential imo)
Form for applying suncream
Form for trips out

Is that what you were looking for (or have I gone of on the wrong track)

Don't forget medical emergency permission (useless really but ofsted very keen on these, don't know about in scotland though).

I always give everyone a booklet of all of my policies too (cos they rarely read them in full in my portfolio) and I then get them to sign to say they have received/agreed to comply by them, (this helps when trying to enforce sickness rules etc).
