Childminders, do you write 'reports' for your children?

If so, what's in them?! We don't have to write any kind of reports for the children in our care in Wales. I'm doing my Level 3 childcare qualification though and my assessor has asked me to make a copy of a child's report that I have previously done (with the child's name blanked out). Obviously I will speak to my assessor on Monday to explain that I don't have one but I would quite like to mock one up to email over to her as I expect that's what she will ultimately ask me to do.

I've tried googling for ideas but childminder and reports just brings up Ofsted type reports.

I do a termly (jan/April/oct) development report which focuses on the prime areas of learning and highlights child's next steps
