Change of address at which childcare is provided
Change of type of childcare being provided, such as changing from being a Childminder to being a Home Childcarer.
CR12:
Change of phone number of the person providing care
Change of name of the person providing care
Change of date of birth (not sure how it's possible to change that!) of the person providing care
Change of address of the person providing care
"any significant event which is likely to affect their suitability or that of any person caring for children on the premises, such as any offences or orders that may disqualify them."
"the name, date of birth, address and telephone number of any person aged 16 or over working or living on the premises"
Person providing care can be the registered childminder, or of an assistant... the Oct 2012 version of the document says "any person working with children".
CR13:
any incident of food poisoning affecting two or more children in the provider?s care
any serious accident or injury to, or the death of any child while receiving childcare
any serious accident or injury to, or the death of, any other person on the premises on which childcare is provided
any allegation of serious harm to, or abuse of, a child committed by any person looking after children at the premises (whether that allegation relates to harm or
abuse committed on the premises or elsewhere), or by any person, where the allegation relates to harm or abuse occurring on those premises.
