cm club: quesionnaire about mindees

A few weeks ago one of you great childminders gave a list of questions to get parents to answer on new mindees. It was things like what was their favourite toy, and when is nap time, favourite foods etc. Could anyone give remind me what sort of questions to put on my list. for Any ideas? Thanks

here's a few:
whether they have been to a childminder before (if not has child ever been left with anyone before)
why are they leaving current childcare
how long they will need place (do they plan to stay in area for long, which preschool/school do they plan to send their child to)
any siblings
any pets at home
special words for toilet/family members etc
do they need childcare in school holidays, inset days, bank holidays, etc.
Does their employer have a childcare voucher scheme (if you would accept these)
Have they checked if they are eligible for childcare tax credits
Do they have a backup plan in case you or your family are sick, need to attend funeral etc
