Applying for OFSTED registration online - whereabouts on the website is the reg form?

I'm trying to register with OFSTED as a requirement for a new nanny job I'm starting soon.

I understand that it is possible to fill in the form online, but I can't seem to find it on the OFSTED website. Has anyone filled this in recently - what heading does it come under? I just seem to be going round in circles!!!

If you need it, I have a guide to completing the paperform and the online form with luck will be very similar - in fact, you could do me a favour and let me know if you spot any differences and if my guide needs updating to reflect anything that has been added on the online form.
