childminder club- help with registration

Ive nearly got my OFTSED file together but Im looking for a form that has questions for parents about childs normal routines. has anyone got such a form? I thought Id do one for children under 1 and one for children over 1.

This is the basic info I like to get:

Childs Name
DOB:

Parent Name, phone, email:

Hours & Days Required:

Start Date: End Date:

Termtime/School Holidays/Bank Holidays?

Has child been to Nursery or CM B4? if not have they been left with anyone (nan, neighbour etc)

Which preschool/primary will child go to?

Other members of the childs family (inc. pets)

Special, favourite or comfort object or toy

Any food/drink not allowed/Food allergies
Eats ? Likes/Dislikes
Meals to be provided by parent/cm

Drinks Water/Squash/Milk
Uses cup/bottle

Breast/bottle fed ? has tried bottles? Quantities & when

Health Issues/Allergies

Anything child finds alarming (dogs, storms, people in spectacles)

Can use toilet unaided/potty training
Uses Nappies (disposable or reusable)

Nap? What times, how long

Cultural/religious or traditional background ?

Favourite Activities, what child does at home

Development/Learning

Any other information from Parent
