did anyone have a doula and does it annoy the midwife?

I'm contemplating homebirth for 2nd DS, am in Holland; but might go to hosp. If you have a homebirth here the midwife comes to check you in labour but she only stays with you continuously once you're 7 cm dilated. This makes me feel quite panicky as I would really like someone with me continously a bit earlier than that. Like from 2 cm!! So am thinking of hiring a doula. But DH thinks the doula might put the midwife's nose out of joint, what do others think? The last thing I would want is a pissed off midwife or to worry about the politics between them of who does what...

Do you speak Dutch? If you do you might be able to get some advice here
