Is there a particular person you need to ask at the hospital? Will they just send them to you, or do they talk them through with you?

Thanks.

Ask your midwife or health visitor and they should be able to get them and go through them with you.

aren't they part of your medical notes with your GP? I would ask your GP. I photocopied mine before they took them away!

I'm not sure - I had DD in a different PCT to the one my Dr and health visitor are in so not sure how that would work..

Call PALS at the hospital where you had the baby
They will send you the form
You may have to pay up to £50 for a copy of the notes
They will also be able to help you if you want to go in and have someone talk you through the notes

do you know how long it takes to get the notes? I'm thinking about getting mine, but DS was born in London and I'm now in Australia and due again in Feb... no point asking if they're not going to arrive in time!

it's usually quite quick, a couple of weeks max
