Any advice on writing a birth plan?

I'm now 38 weeks pg and, although I've thought about how I'd like the birth to go, I still haven't got round to putting it in any order like a birth plan. I really have no idea how to do this and all the books I've looked in give different advice. At this rate I'll be handing the midwives an essay!! Has anyone got any suggestions or examples??

i wrote the birth plan on the night i went into labour!!
I passed it to the midwife and because of difficulties, the plan went stright out of the window!!!

I am planning a home birth. How necessary is it for me to write a birth plan?

I wrote a lovely detailed birth plan. When I told the midwife he said 'that's nice' and promptly totally ignored it.

JoD, not really necessary I don't think as you will be able to talk to the midwife who will come to your house beforehand, unlike hospital births where you may not have met the midwife before.

Zubb, thanks for your reply. The midwife who will attend to me will be the one on call when the baby comes, so will be one of a team of 6 and not necessarily the midwife I have been seeing regularly. She will also be accompanied by a midwife from another team, who I will definitely not have seen before.

not going to comment about the content - but don't do what I did - which is never show it to teh midwives. They didn't ask (which I think they should have done), and I was to shy (stupid) to thrust it into their hands. If they don't ask - be assertive.
