Shoulder dysplasia risks, what did you do?

Hello all, I've been told my baby is over 95th centile. This carries the risk of shoulder dysplasia which has me worried.
I've got two other children, both were showing at 75th centile, 8.11lbs and 9.2 lbs.
I've had a PPH requiring blood transfusion with them both.
I'm worried about myself and the baby during this birth.
I'm no spring chicken at 37 and already overweight so also worry about recovery from a caesarean.
Not to mention my 9.2lb baby is only just turning 1 the month before new baby is due.

Any similar situations you can think of to give me some advice?? I just don't know what kind of birth to opt for, apparently caesarean comes with greater risk of PPH aswell, although I'm 2 for 2 so far so quite likely with this one too.
