The hospital one I have is very much do you want this or that option, which is a bit black & white for me

I was struggling with the format, as I wanted it to be a positive statement about the sort of birth I would like, not a list of do's & don'ts. These have really helped & I'm sure I can nab suitable words from all of them

Glad you'll be able to use something from them all. I had a really good one 1st time around, but won't be suitable for you, as you are vbac, but, like you say, it was very positive and gave the midwives their place, as such.
