Heart condition and birth plan

Hello
I am 26 weeks and am getting increasingly anxious about giving birth
I have restrictive cardiomyopathy and am being monitored regularly throughout. I have discussed with my consultant prior to pregnancy about whether I have children and if I could have a natural birth etc
My tests have stayed the same and they are still saying they are happy for me to give birth naturally but nobody has really discussed a birth plan with me, I don't really have any idea about it and I'm really anxious over my heart condition so I'm getting very concerned about giving birth and was wondering if and elective c section would be my best option, I don't want to end up in labour and be panicking and putting more pressure on my heart
I don't really know who I even discuss this with, my midwife hasn't really mentioned anything about a birth plan or what options I have

I'm 38 weeks. I don't have a heart issue as a complicating factor. The most discussion I've had with the midwife about a birth plan was at the 36 week appointment where she said I should write a birth plan for my next appointment - super helpful... not!!!

I'd suggest you start thinking about what you want and then propose those things to the medics. They don't seem to be very pushy about anything (except the last midwife who told me not to go for a midwife lead unit?????? But she wasn't very nice)...
